I don't understand the meaning behind this emoji. Is it relating to using the bathroom facilities?

Yes you are true. According Webster dictionary:
Definition of the call of nature
      humorous
: the need to use a toilet
ex: He had to leave suddenly to answer/obey the call of nature.
